Practices
We consider protection of your privacy an
integral part of our high clinical standards. We are all committed to ensuring
our patients can confide in us with confidence that any information they share
will be protected. We know that you
place a lot of trust and confidence in us and we work very hard to exceed your
expectations. What type of information are we talking
about? In
the course of receiving your dental care, you give us a lot of information.
This can include details about your health history, dental condition,
address, social security number, home and work phone numbers, insurance, credit
card and checking account information along with other personal or financial
information. We consider any
information that specifically identifies you, your family, or details about your
health or financial background to be protected information.
Why do you need this information?
We need this information to
ensure that we are able to provide the best possible dental care.
Personal health and dental information is essential to give our dentists
a thorough history of your condition so that they can accurately diagnose and
treat dental problems that may arise. Your
personal and family information is essential to allow us to contact you to
follow up on your care or contact your family (or responsible party).
It also allows us to help you keep track of your appointments and assist
you in managing routine dental maintenance and prevention of disease.
We also need information about insurance or third party payers so we can
help you best utilize all your resources to pay for dental care for yourself and
your family. We may also ask for
financial information to assist you in applying for financing for dental
services. All of this information
helps us to give you the best dental services possible. What
do you do with this information? Mostly,
it just stays in our office for use by our clinical and administrative staff
during your appointment. We can also
use this information to contact you; to coordinate your care with other health
care providers, pharmacies or hospitals; to file for reimbursement from third
party payers; to help you apply for a loan; to keep additional records in case
you need copies; or for other uses related to your dental care.
We do not buy, sell or trade your information to direct marketing
companies nor do we release your information to any person or organization that
is not involved in your dental care except as required by law or for collections
of delinquent accounts. We
reserve the option of releasing personal information in the event of a general
public health threat that could harm you, your family or other members of the
community. We also may release
protected information to a government authority in the event we reasonably
believe you or a member of your family are a victim of abuse, neglect, or
domestic violence. How do you protect this information?
We use a variety of methods
for protecting your information. Since
this information can be in electronic or paper form, different methods are
required depending on the medium. We
shred or burn paper documents that have protected information.
For electronic information we use a combination of data encryption,
firewalls, password protected networks, anti intrusion software and advanced
network security protocols to prevent deliberate or accidental compromise of
your information. We are constantly
upgrading and updating our electronic protection to keep up with new technology
and threats to your privacy. We also
back up patient data every few hours to protect against data loss. Who manages your privacy program?
